1. If you are a U. S. or Canadian Citizen: Please complete this form and return it to our office IMMEDIATELY. We will be obtaining group visas for passengers going to
Egypt and Jordan. Visas will not be sent to you. 2. If you are a citizen of any other country: We will be able to obtain your visa for Egypt and/or Jordan; however, you may need to obtain your own
visa for Israel. NOTE: Multiple entry visas will be required for some nationalities. WE CANNOT APPLY FOR YOUR VISA UNLESS YOU COMPLETE EACH LINE OF INFORMATION BELOW.
